PIM approvals not always going through

We have a few cases when a PIM approver approves a PIM request, the approval never actually goes through even though the portal says it does, and the request will still be there, and the approver will sometimes need to approve it multiple times to go through.

Seems to have happened after recent GUI changes. How can we troubleshoot this?

Date and time of the approval (including time zone)

Request ID(s) of the affected PIM requests

Whether the issue occurs with Azure resource roles, Microsoft Entra roles, or both

Number of users and approvers affected

Browser and browser version used by the approver

Screenshots showing the approval was completed but the request remained pending

If available, please also share any relevant entries from the PIM activity logs or Audit logs, including any correlation IDs associated with the approval action.

Please share any sensitive information, such as request IDs, correlation IDs, and screenshots, via the Private Messages tab.

Once we have these details, we'll review them and continue the investigation.
